AETNA INSURANCE:

I’m a provider for Aetna PPO. If you have Aetna PPO I can bill your insurance directly. To be sure your Aetna plan covers my services, call the number on the back of your insurance card to check. Please note: copay's, coinsurance & deductibles apply. 

CIGNA INSURANCE:

I accapt most Cigna Insurance plans, PPO & HMO. To get pre approved so I can bill your insurance directly go to

UNITED HEALTHCARE:

For all other healthcare plans payment is due at the time of visit. I will then provide you with a Superbill for you to submit to your insurance for reimbursment.

 

Travel Fees:

A travel fee will be applied for home visits, this is an out of pocket cost, not covered by your insurance.

• $75 up to 45 minutes round trip from 90034

• $100  up to 60 minutes round trip from 90034

• $150  up to 90 minutes round trip from 90034
